MEDITECH Required Building Access Form & Fitness Waiver

The undersigned is requesting entrance the property at the Corporate Center at 550 Cochituate Road in Framingham, Massachusetts.

ASSUMPTION OF RISK
The undersigned may utilize the Fitness Room and its Equipment. The Equipment is available for use by employees of tenants and occupants of the Building. The Building does not require any pre use physical or medical qualifications for individuals utilizing the Fitness Room. Individuals utilizing the Fitness Room do so at their own risk.

The undersigned hereby acknowledges that his or her use of the Fitness Room is at his or her own risk and agrees not to seek any claim or recovery against MEDITECH or its staff for any injury or adverse condition that may result to the undersigned as a result of his or her use of the Fitness Room or the Equipment located there.

Access Cards are needed to enter the facility after hours and on the weekends. Card reader doors are located at the front lobby and rear lower parking lot entrance.

The form must be completed and returned to the building receptionist or faxed to the attention of Framingham Operations at 781-821-2199. You will receive your Building Access Card in approximately 3 days after approval. Thank you.
